12-month Short-term Medical Insurance

Are you between jobs? Or perhaps a recent college graduate looking for that first job? Then chances are you're in need of temporary medical insurance until you become part of an employer's plan.

The Short-Term Medical Plan provides comprehensive, temporary medical coverage at a reasonable price. It's an affordable plan that gives you peace of mind knowing you won't have to worry about the cost of health care. You select the coverage period that fits your situation. Features include:

• Choice of coverage periods

• Eligible for one renewal period

• Billed monthly

• Pay premiums monthly, up to 12 months

• Option for up to 36 months of coverage (Three 12 month coverage periods, with continuous claims coverage. (Please see details.)

• No PPO or HMO, choose any doctor or hospital

• 24-hour coverage worldwide

• $1,000,000 Lifetime Maximum per Insured Person

A new kind of Temporary Major Medical Insurance Plan that is flexible and satisfies your medical insurance needs for up to 12 months at a time.

You can pay for the coverage you need now, or pay it monthly for up to 12 months, so it's easy on your budget.

If you continue to need major medical insurance at the end of the 12 month period, you can apply for another 12 month coverage period.

You can apply for up to three consecutive 12 month coverage periods.

Here's how it works:
Once you and your dependents have satisfied the eligibility requirements on the application form and have paid your first monthly premium, the Certificate of Insurance will be issued for a 12 month coverage period.

You can pay your premium by monthly installments (includes an administrative fee); or you can pay by automatic pre-authorized bank withdrawal or Master Card, Visa and Discover cards.

When your coverage period is almost over, you will receive an application form to apply for another 12 month coverage period.

If you re-apply within 30 days prior to the end of your coverage, your coverage and monthly installments will not be interrupted.

Furthermore, any condition(s) for which benefits were paid during a certificate period will not be subject to pre-existing conditions limitations during any subsequent certificate period, provided the enrollment form is received by HPA, Inc. on time.

However, any condition(s) that were excluded because of a pre-existing condition under the prior coverage period will continue to be subject to the pre-existing conditions limitation under the following coverage period.

Benefit per insured person for each 12-month coverage period

• FIRST you pay the Deductible: $250 of Covered Expenses

• The PLAN then PAYS 80% of the NEXT $5,000 of Covered Expenses

• The PLAN then pays 100% of the REMAINING Covered Expenses up to a Lifetime Maximum of $1,000,000

Catastrophe Major Medical Insurance

Why You should consider Catastrophe Major Medical Insurance? You never know when a severe illness is going to strike, or when a potentially devastating accident will occur. What you do know, however, is that they happen to someone – somewhere – each and every day.

$2,000,000 in Protection: In the event of a covered illness or injury, the Catastrophe Major Medical Insurance plan can pay up to $2,000,000 in benefits per person, per 5 year benefit period while insured under the plan.

Affordable Group Rates: You and your family can benefit from the affordable group rates available with this plan. Premiums increase as the member enters a new age bracket. Premiums are not guaranteed, and can be increased on a class-wide basis.

Can Help Preserve Your Savings and Right to Choose:
By helping you avoid large out-of-pocket expenses, this plan helps preserve the assets you have worked so hard to achieve. And because this plan allows you to use out-of-network health care providers and facilities, it helps remove obstacles your current basic insurance plan may place on your freedom to choose.

Pays for Private Duty Nursing and Home Health Care: Catastrophe Major Medical Insurance also provides benefits to help pay for private duty nursing care and home health care services.

Survivor’s Benefit: Even if the insured alumni passes away, insured dependents will retain their coverage as long as they continue to meet all other eligibility requirements and the group policy remains in force. (However, rates may be adjusted depending on the survivor’s age.)

Pays for Prescription Drugs: Once your deductible is satisfied, this plan will pay 100% of all covered drugs prescribed to treat your illness or injury, or pay the difference between actual cost and what your current health insurance will provide for the balance of the Benefit Period.

Disability Income

At a Glance

• Monthly benefit if you or your spouse are disabled and unable to work

• 70% of your income up to a maximum of $10,000 monthly benefit

• Your choice of waiting period; 30, 60, 90 or 180 days

• Your choice of benefit period; 2 years or to age 65

• Guaranteed renewable

About the Alumni Disability Income Insurance Program
The program is underwritten by Hartford Life Insurance Company and administered by Marsh Affinity Group Services.

There are two insurance plans to choose from:

Plan I: (for alumni only) . . . if you are disabled by a covered accident or illness, you will receive benefits beginning on the day after your waiting period is satisfied and continuing up to age 65 (up to 24 months if disabled after age 65).


Plan II:
(for alumni and spouses) . . . if you are disabled by a covered accident or illness, you will receive benefits beginning on the day after your chosen waiting period is satisfied and continuing for up to two full years. Spouses can receive benefits up to a maximum of $2,000 per month.

How Disability Insurance Works
Disability Insurance covers you against the risk that you will be unable to work because of an accident or sickness. The policy pays benefits equal to a portion of your wages when you can't work because of long-term disabling illness or accidental injuries that happen on or off the job.

You can select benefit amounts, how long you must be off work before benefits are paid, how long benefits will be paid, and other plan features.
Why You Need Disability Insurance
A disabling accident or illness can happen in the blink of an eye. That disability could place you out of work. If you have to stop working, chances are your income will stop too. But it doesn't have to if you protect yourself with disability insurance.

Under the Disability Income Plan, you receive a monthly benefit that you select (up to the specified percentage of your income). With this benefit you can keep up with your living expenses until you get back to work.

Don't let a disability put an end to your income. Disability insurance is an essential part of everyone's portfolio.

Long Term Care Insurance

At a Glance

Long term care insurance pays many expenses not covered by Medicare and helps prevent you from having to "spend down" your assets, a prerequisite for Medicaid assistance. More importantly, it helps protect your freedom to choose a nursing home facility or home health care provider.

The Long Term Care Program is available to University of New Mexico alumni between the ages of 55 to 72. The Program is underwritten by Metropolitan Life Insurance Company and Prudential Life Insurance Company, both companies have top ratings from AM Best.

All alumni are offered a 5% discount if they apply for coverage through either Metropolitan or Prudential Insurance Companies. This discount is available only to alumni of the University of New Mexico.

Life and Common Carrier Insurance

Exclusively for new graduates!

The UNM Alumni Association’s Graduation Gift to graduates under the age of 30 is the opportunity to obtain $25,000 group term life insurance with up to $75,000 of common carrier accident insurance.

Your first year after graduation will be a time of creating your own financial security.

This life and accident insurance coverage is intended to give you a good start on creating that security.

You will not be called on by any salesperson regarding this insurance and you are not required to buy any additional coverage.

You may name anyone you wish to be your beneficiary.

As an added benefit, the UNM Alumni Association and administrator have worked in conjunction with the insurance company to guarantee that you will be able to continue your coverage – if you wish – at the end of this gift period.

At the end of the gift period, you are guaranteed the opportunity to remain insured under the Alumni Association Life Insurance Plan for $25,000 (plus up to $75,000 of common carrier accident coverage) on a premium paying basis.

In addition, for the five years following the effective date for the $25,000, you will be guaranteed the opportunity to increase your coverage by 20% ($5,000) annually, so long as you continue to accept each increase and pay the required premium.

Your common carrier accident benefit automatically increases with your life coverage so that it is always equal to three times of your life insurance benefit, subject to one million dollars of common carrier accident coverage.

Your coverage will become effective on the first day of the month following receipt of your registration form.

The information presented here explains the general purpose of the insurance described, but in no way changes or affects the policy as it is actually issued.

In the event of a discrepancy between the information presented here and the contract, the terms of the contract apply. Complete details are in the Certificate of Insurance issued to each insured individual.

This program is not available in all states.

Plan Features

Both you and your spouse need life insurance to protect your family against an unexpected death. If he or she were to die unexpectedly, a similar financial hardship could occur. Your spouse (under age 65) is eligible to apply and receive these affordable rates available through the University of New Mexico Alumni Association.

The University of New Mexico Alumni Association serves its members from the oldest building on campus, Hodgin Hall.

The Association and its professional staff in the Alumni Relations Office coordinates activities designed to inform and involve UNM’s 143,000 alumni in every state and 89 foreign countries.
